Ploughshares Summer Program Forms

School is close to the end and we are creeping up on summer. We need to get summer forms from families as soon as possible, as we need to get all paperwork together for our summer licensing. The link to the forms is below or you can ask a teacher for a hard copy at pick up. With these forms we also need a current (with in the last 12 months) physical for all children. This is required by the health department.
If your child has meds, I will bring them over from Lincoln Eliot along with the current action plans. FYI Action plans for medical conditions (allergies, athsma, etc) are only good for a year. We will need new ones for the school year!
